SCENARIO
??Uncle Jim’ is a 59 year old Aboriginal gentleman who lives with family and friends at Port Augusta. He is semi nomadic; often spending periods of 4 months or more in other locations with relatives from his
own kinship group. He has a very close female friend in Port Augusta with whom he lives when there.
Systems Review
Neurological
Uncle Jim reports that he had a stroke 3 years ago that resulted in him having weakness and immobility of his left hand. He is alert, oriented to time, place and person, and demonstrates a good sense of
humour. He speaks slowly and is slightly forgetful at times.
Endocrine
Uncle Jim has diabetes which he manages with a diabetic diet. He reports that does not take insulin or any other diabetic medications. However, when questioned he does not appear to have a very good
understanding of what diabetes is, why he needs to understand this condition and his role in the management of the diabetes. He says that his doctor told him to “keep an eye on his sugar levels”.
Respiratory
Uncle Jim does not smoke but reports shortness of breath on exertion. He informs you that he has been admitted to hospital twice during the past couple of years as a result of chest infections.
Cardiovascular
Uncle Jim has hypertension (185/100), his pulse is fast and bounding 100 beats per minute. He could not remember if he has a management plan was this or what he is supposed to do about this. He does not
drink alcohol anymore, but says he used to drink “a lot” in his younger days. Jim is very thin given that he is 170cm tall and weighs 60 kgm.
Musculoskeletal and Integument
He has paralysis of his left hand. There are scars from ulcers on is left lower leg and you note that his left toe is very inflamed and red.
