You are required to complete and present FOUR different forms related to the Payroll Process and where specified based on information provided in Assessment 1. These forms are:

PAYG Summary
BAS Statement
Pay slips (to be done in MYOB and uploaded to moodle)
1. TAX FILE DECLARATION FORM
use the four employees information provided in Assessment 1 activity 2&3
Staff name ( surname provided in Activity 2/3)
Address of each staff (choose any)
Your business ABN- 14 digits start with 0
Your business address (choose any)
All your staff are Permanent resident (for tax purposes only)
You will need to use a combination of the information provided below as well as the information in Activity 2/3 to fill up other relevant information needed, please use x when filling up the form that require
options to choose from. The information below supersedes that of Activity 2/3. E.g. if activity 2/3 implies that the employee should receive a tax free threshold but the information below mentions this is not the
first job, than you must follow the information below when populating the TFN declaration form.
Batman has 2 jobs. he earns $30,000/- in his first job and $35,000/- in his second job which is part time. Help him to fill out his tax file declaration form for his second job and what advice will you give him.
Spiderman is an australian resident for tax purpose who has a dependent spouse who is residing overseas. Help him to fill out his tax file declaration form.

Green Lantern has changed his name since he last filled in his tax file declaration form. Help him to fill out his tax file declaration form.
BUSINESS ACTIVITY STATEMENT- PAYG TAX WITHHELD SECTION OR OTHERWISE CALLED W1 SECTION
You will need to calculate the following in order for you to fill up the form appropriately;
Calculate the staff 12 weeks gross pay
Calculate the staff 12 weeks tax withheld
Fill up section W1, W2 and W5 and remainder of form.
You must use the information provided in Activity 3.
2. PAYG PAYMENT SUMMARY STATEMENT
Calculate the annual gross pay of your 4 staff
Calculate the annual tax withheld for all your staff
Calculate the annual superannuation paid to all your staff
Indicate total number of payment summaries issued- on a normal scenario, the number of staff you have in your payroll is the number of summaries your company will be issuing
You must use the information provided in Activity 3.
PAYSLIP- Issue payslip to each employee based on activity 3
To complete this task, you need to do the following;
Provide the following information;
Employee classification, level, address. Commencement date and relevant information needed, hourly rate, Public Holiday, voluntary and statutory deductions, superannuation.
Employer details, address and ABN number (as per Tax declaration form that you made for this Portfolio activity).
